Before your DOT physical
Prepare for your visit, choose the right service and know what to do after your exam.
DOT physical: $85. Drug test: an additional $85. The urine test included in the physical is not a drug screen.
Book an $85 DOT Physical Call 704-461-1836
1. Book the service you need
New to Beyond Wellness? Start with the DOT physical booking link. Follow Jane’s prompts to choose a time and sign in or create your patient account. Creating an account alone does not reserve an appointment; finish the booking and check for confirmation.
Returning patient? Use your existing Jane account. If you cannot access it, contact the office before creating a duplicate account.
Need drug testing too? Call 704-461-1836 before your visit to arrange it and discuss any employer paperwork. The drug-screen listing currently directs patients to contact the office.
Sending drivers for a company? Request company setup before arranging company-paid visits.
2. Bring your information
- Your driver’s license or identification and current medical certificate, if you have one.
- A current list of prescription medicines, over-the-counter medicines and supplements, including doses.
- Glasses, contact lenses or hearing aids you use.
- Any employer instructions and medical records requested by the examiner.
- If you use CPAP, take insulin, have a relevant medical condition or were asked for a specialist report, call ahead to confirm which documentation is needed.
Keep taking prescribed medicines as directed by your treating clinician. Do not change treatment to influence an examination result. Review any forms or instructions sent by the office or made available in your Jane account.
3. Find the office

4. Know your next steps after the exam
Ask the examiner about your result, any follow-up and the actual expiration date of your certificate. Certification is not guaranteed, and the interval may be shorter than two years.
CDL/CLP drivers: follow your licensing state’s instructions and confirm that its medical-certification record is current. Non-CDL drivers: paper-certificate requirements differ. See our North Carolina medical certificate and NRII guide.
Save your actual expiration date and plan renewal ahead of time. Do not assume a website booking or an account registration renews your certificate.
