How company setup works

  1. Send a business inquiry. Share your contact details, estimated visit volume and services needed.
  2. Review arrangements with our office. Confirm which services we can provide, how visits will be paid for and what instructions drivers need.
  3. Send drivers using the agreed instructions. An inquiry alone does not approve a billing account, establish credit terms or reserve appointments.

Services and pricing

  • DOT physical: $85.
  • Drug test: an additional $85. Contact the office to coordinate the test and any employer instructions before sending a driver.
  • For other occupational health needs, ask the office about availability and arrangements.

A physical’s routine urine test is separate from a drug screen. Employer payment arrangements, any volume pricing and any on-site services require office confirmation.

    Do not include driver names, medical information, Social Security numbers or payment-card details. This request does not open an approved billing account or book appointments. Our office must confirm arrangements before you send drivers.

    Before sending a driver

    Confirm the service requested, who will pay and any employer paperwork with the office. Drivers should bring their identification, current medication list and relevant medical documentation requested for their exam. Medical information should be handled through the appropriate patient process, not this business form.
